The obligatory rules:
1. Only one survey form submission per category per person, please!
2. Your time is valuable, so we’ve limited the number of voting categories on our ballot. While you can choose which categories (see below) and questions you would like to complete, please be generous with your wisdom and experience and fill out at least 10 questions throughout the survey. Better your chances of winning a prize by completing the entire ballot: For each of the five category survey forms you complete, you will receive one entry in our prize giveaway.
3. When questions concern a store or other business, we ask that submissions be for locally/regionally owned establishments.
4. You must provide a viable email address at the bottom of each survey form you complete so that we can contact you if you win. Your email will only be used to contact you in the event you win.
5. All entries must be received no later than May 15, 2012. Results of our Golden Teddy Awards reader picks survey will be published in the July 2012.
